40 hours a month, depending on location and availability-Mon-Sat- Chula Vista/East Lake
$20.50 per hour plus $2 for gas (fixed rate)
The Community Facilitator performs duties under the direct supervision of the Director of Living Resources. The qualified candidate has working knowledge and a philosophy of community-based instruction.
Qualifications:
A commitment to the agency’s primary vision, mission, and values as they are to be actualized in the lives of the adults we support.
A belief that all adults have the right and ability to live in the community.
Proof of BA/BS in human services-related areas or a combination of practical experience and education.
Valid CPR and First Aid training.
Must be able to pass a drug screening and background check.
California driver’s license and proof of automobile insurance.
Reliable transportation.
A willingness to pursue training and certification deemed necessary based on the needs of the individual(s) supported.
ESSENTIAL FUNCTIONS OF THE JOB INCLUDE:
Training, support, and monitoring of the individuals supported in the community and home settings.
Completing Living Resources required paperwork, i.e., progress notes, timesheets, etc.
Develop and maintain a Google Calendar online.
Implementation of ISP goals.
Schedule, attend, and participate in 30-Day Reviews, Semi, and IPP meetings.
Attend and participate in in-service training.
Mandatory attendance at Living Resources required meetings, including all-staff and team performance evaluations.
Submit required verifications for employment.
OTHER DUTIES OF THE JOB INCLUDE
Assistance in supporting an individual’s attendance at agency functions, self-advocacy meetings, etc.
Other assignments, responsibilities, and duties as deemed appropriate by the Director of Living Resources.
PHYSICAL REQUIREMENTS:
The job will require intermittent driving daily in your vehicle.
The job may require assisted lifting or balance control for individuals as needed when using stairs, uneven ground, curbs, street crossings, and when entering or exiting vehicles.
Supporting, lifting, or carrying may occur daily. (If the weight is beyond your physical limitations, more than 20 pounds, and if the task is done repeatedly, personal or mechanical assistance are required.)
The job requires frequent simple grasping, occasional power grasping with either or both hands,s and some fine manipulation of paperwork.
Sitting, walking, bending, standing, and stair climbing will occur daily.
You may be exposed to biohazards while assisting with personal hygiene issues, CPR, or First Aid. In all cases, you are required to wear gloves and wash your hands thoroughly with soap and water, waterless liquids, and/or wipes.
Noise exposure varies from low to moderate levels.
Repetitive use of hands is necessary for occasional computer work.
Seasonal temperatures can vary from cool and dry to hot and humid, with occasional exposure to the sun.
