The Program: The Fred Finch Specialized Wraparound Program works directly with the Regional Center to provide wraparound services. Participants served demonstrate behavior challenges that pose barriers to living successfully in natural home environment and impair their ability to create healthy, lasting relationships with others. Therapists make an assessment and work with youth, treatment providers and families to develop and implement a treatment plan to work with families to preserve the stability of the home environment. The goals of the program are to address the youth’s developmental and/or mental health needs, diminish negative social behaviors, and to preserve the family. The program employs family-centered, strength-based practices and a culturally competent approach.
The program offers the following services: assessment and intervention, individual and family counseling; case management; caregiver/parent coaching; crisis response; linking family/youth to natural and community support; and access to psychiatric support and medication.

The Position: Full-Time Care Coordinator based out of our Spring Valley location. This position is exempt/salaried
JOB SUMMARY:
The Care Coordinator is responsible for providing high quality, trauma-informed therapeutic services and/or case management to a diverse population of at risk youth, young adults, and families who may be experiencing barriers to receiving office-based behavioral health services. These developmentally appropriate, individualized services may occur in the community, school, office, or residential setting.
REQUIRED QUALIFICATIONS:
M.A. Degree in Social Work or Counseling from an accredited college or university.
Registered intern or associate with the Board of Behavioral Sciences or ability to obtain registered status within first 6 months after hire.
Certified to conduct a CANS/ANSA assessment within 90 days of employment if the work assignment
Demonstrated knowledge in the areas of diagnostic assessment, individual/family/group behavioral treatment methods for target population of program.
Fluency in prevailing language(s) of the community strongly preferred.
