I have an existing HTML web form that currently dumps its data to a hidden e-mail account. I'd like to convert that destination away from the mailto and into an empty and waiting SQL database (hosted by GoDaddy on Linux running PHP 5.x).

I know this is a standard, run-of-the-mill task, but it's beyond my abilities as a visual web designer. I need someone (preferably today) to bang this out by modifying the existing HTML (which I'll provide), test it, and confirm it's functional and stable.

* Review a range of existing web designs and group them into named categories.
* Describe the categories in ways that other designers can create new, similar designs.
