Job Description
Job Tittle: Life Skills Counselor (Job Coach)
Main duties: As a direct service provider, the life skills counselor (Job Coach) will assist the service recipient in the development of functional skills through training in small heterogeneous groups in the community and natural settings, consistent with the service recipients’ Individual Service Plan (ISP). The life skills counselor will provide service recipients with high quality community integration training, volunteer opportunities and employment development and exploration.
Qualifications: High school diploma, or at least one year of previous experience working with individuals with developmental disabilities. Experience is important, but motivated & enthusiastic individuals, who are eager to work in a rewarding social services area will be trained by our staff.
Responsibilities: Conducts client contacts and schedules and weekly activities consistent with the clients’ Individual Service Plan (ISP) long and short term goals. He/she will be included in all planning team meeting, and will maintain adequate and accurate clients’ case files, including planning team meetings meetings. Life skills counselor will also assist consumer and other members of the planning team with the intake and assessment process if required by the program.
