Job Description

Job Tittle: Independent Living Resource Counselor

San Diego New Horizons is a social services agency, which provides independent living services (ILS) to individuals with developmental disabilities. We are looking for motivated individual to join our team as direct service providers.

Main duties: As a direct service provider, the ILS-Resource Counselor will assist the service recipient in the developing of functional skills training necessary for independent living, consistent with the service recipients’ Individual Service Plan (ISP). The ILS Resource Counselor will also assist the client in identifying community and/or medical resources in order to maintain a self-sustaining independence in every aspect of his/her life.

Qualifications: High school diploma or previous experience working with individuals with developmental disabilities, bilingual abilities in Spanish & Tagalog highly helpful. Individuals who lack experience in this field, but are eager to work in a rewarding social services area, will be trained by our staff.

Responsibilities: Conducts client contacts and schedules of weekly training activities consistent with the clients’ independent support service (ISP) long and short term goals. He/she will be included in all planning team meeting and will maintain adequate and accurate clients’ case files, including quarterly contact meetings and related documentation. ILS Resource Counselor will also assist consumer and other members of the planning team with the intake/assessment process if required by the program.
