*Must be able to read and speak English fluently*

1) Are you between 55-90 years old?
2) Have you been diagnosed with Alzheimer’s disease?
3) Are you currently taking one of these medications; Aricept, Exelon,
Reminyl/Razadyne?
4) Do you have a caregiver?
5) Can your caregiver accompany you to each of your visits?
6) Do you have an adequate place of residence that is not a nursing home?
7) Are you pregnant, nursing or plan to become pregnant in the next year?
8) Have you had alcohol or drug abuse in the last year?
9) Have you participated in clinical trials within the past 30 days?
